Appointment Request

I am a seasoned clinician and out-of network provider for all health insurance plans.  

Please note:  I do not bill health insurance plans for services.  If requested, the documentation required for the client to submit a request for reimbursement to their insurance company will be provided.  Any reimbursed expenses will be paid directly to the client.  

Session fees are based on a 50 min hour.  All fees are payable by cash, credit, HSA or check and are due at the time of service.
